There is no protein store in the body equivalent to fat or glycogen; every gram of protein is doing a job. Deprived of dietary protein, the body breaks down skeletal muscle to supply the amino acids that cannot be skipped, which keeps the essential machinery running at the cost of the structure. Blood albumin falls, and because albumin holds fluid inside blood vessels, water leaks into tissues — the swollen belly and limbs of kwashiorkor. Immune function, wound healing and gut lining all depend on continuous protein synthesis and all degrade together. If calories are adequate but protein is absent the decline is slower and stranger than plain starvation; if both are missing, the outcome is the same as starvation.
